Community Health Assessment (CHA)
A community health assessment (CHA) refers to a state, tribal, local, or territorial health assessment that identifies key health needs and issues through systematic, comprehensive data collection, and analysis.

Minnesota community health boards have been required to engage in a community health improvement process—beginning with a community health assessment—since the Local Public Health Act was passed in 1976. In Minnesota, community health boards perform community health assessments for the jurisdictions they serve. After completing the community health assessment, public health staff in partnership with the community prioritize the health issues and/or factors impacting health to collectively address during the planning process. Minnesota Department of Health may ask community health boards to periodically rank their communities' priority health issues, to provide a view of statewide priorities, and to fulfill the requirements of Minnesota statute. Community Health Improvement Plan (CHIP)
A community health improvement plan (CHIP) is a state requirement every five years and utilizes the results of community health assessment activities and the community health improvement process. The CHIP helps public health organizations strengthen internal and external assets and identify and close service gap areas, and learn about community resources. This plan will provide a strategic direction to help public health and community partners close the gap in mental health and substance abuse.
The Community Health Improvement Plan (CHIP) is a long-term plan that helps identify Le Sueur County health priorities and helps identify gaps and barriers in a systematic process. Community Conversations, data, and stakeholder input prioritize five health areas. The CHA and CHIP come together with a Strategic Plan and Quality Improvement Plan to help improve health.

The Community Health Improvement Plan results and process can be found here. CHIP updates post to social media platforms and outreach efforts may include community and partner engagement.
The CHIP is is for community partners, businesses, stakeholders, non-profit organizations, community members, and public health to help improve Le Sueur County resident health and for strategic planning, volunteer opportunities, support groups, policy development, grant funding, reducing community barriers, fundraisers, local engagement efforts, and more. The CHIP is a community effort and more partner involvement can have a positive resident health impact.
The Quality Improvement Plan outlines the goals and strategies the Le Sueur-Waseca Community Health Board will undertake to conduct quality improvement measures in the Public Health departments.
