What is Needed for a WIC Appointment?
Proof of Medical Assistance or Income, like your MA card or a paystub
Proof of identification, like your driver's license or other government ID.
Infants and children need identification such as a crib card or social security card.
Proof of residency like a piece of mail with your name on it
If your child has medically prescribed formula or supplements, please bring a prescription signed by your doctor.
