Applications need to be completed using BLACK ink only OR complete application online and print single sided. *Do not sign DS-11 application until you are in front of a Passport Acceptance Agent*

Passport (16)

Proof of citizenship is required.

Examples include: 

  • Certified Birth Certificate
  • Certificate of Naturalization
  • Report of Birth Abroad

If you were born in Minnesota and do not have your Birth Certificate we can print one in office for an additional fee.

Passport (17)

If two parents are listed on child’s birth certificate but only one parent can attend the passport appointment additional documentation is needed. Examples include but are not limited to: DS-3053 form, Death Certificate, Certified Court Order of sole legal custody. For more options please visit travel.state.gov
